  • Once you have accessed the next screen, please review your completion status. If you are incomplete in any of these sections, please contact the Board at info@vahealthprovider.com for assistance in meeting your statutory requirement.

    Procedure for Updating Your Questionnaire

    In order to save any changes you make to your questionnaire, you must update that information by clicking on either the "Previous Section," "Next Section," or "Update Information" button at the bottom of the screen for that section of information AND proceed to the Verify screen to review the revisions and then click on the "Submit Verification" button.
