Frequently Asked Questions (FAQs)
  1. What happens if I don't respond? It is a condition of maintaining a license in Virginia. No one can be excused from the statutory requirement.
  2. How do I get help? Email us at info@vahealthprovider.com or you may contact us at 804-367-4444, Monday through Friday from 8:15-5:00 for further assistance.
  3. Where do we get a password? It is in your notification letter.
  4. What if I forget my password? Contact the Board of Medicine Call Center at 804-367-4444 or at info@vahealthprovider.com to request your password to be emailed to you (if you previously established a non-emergency email address with the Board at the time of initial completion of your questionnaire) or mailed to your address of record.
  5. Why do I have to change my password? To keep your questionnaire secure.
  6. I changed my password. Why won't it go any further? Hit refresh and try again.
  7. Can I complete over the telephone? No.
  8. Can this be done from home? Yes.
  9. Is any of the information personal? All information is for public display with the exception of the amount of any malpractice paid claims and emergency contact information (ECI). The questionnaire deals with questions regarding your professional career.
  10. What if a malpractice payment is sealed? The law doesn't make a provision for sealed malpractice payments. Consult your attorney.
  11. If I don't have hospital affiliations can we leave it blank? Yes. Answer no and hit the "update" or "next section" button to save your answer and move on to the next section.
  12. VA Board Notices and Orders You should be answering "true" if you agree with what is displayed - either "I have not been the subject of a Virginia Board of Medicine Notice or Order" or "I confirm that I have had the opportunity to review copies of the Notices and Orders..." If completing online, you will be prompted to "click here to view notices and orders" and you are presented with a chronological list you can click on and view scanned documents. If you are completing a paper questionnaire, a copy of your notices and orders will be attached to the questionnaire.
  13. Does the comment section in malpractice take 300 letters? No 300 characters including spaces.
  14. Is my Address of Record available to the public? It is not included in the profile however, it is public information. Your address of record is already available under "License Lookup" at the Department of Health Professions website (www.dhp.virginia.gov) . The street address of Address of Records will not be published on "License Lookup." It is permissible for an individual to provide an address other than their residence, such as a Post Office Box or a practice location as their Address of Record.
