Action Taken: September 2012: Reprimand: I was given a Reprimand for failure to address a patient's back pain post cardiac catheterization. I was unable to order and outpatient narcotic for the patient and could not locate assistance to complete this endeavor. The patient had tylenol ordered which was not given and also Morphine sulfate IV which was given and relieved his pain. He was discharged to home that afternoon without difficulty. I have an EEOc pending with respect to the temporary suspension and Reprimands.

Action Taken: September 2013: Reprimand: I refused to staff a complex EP patient with recurrent VT and ICD discharges on Amiodarone with a PA in clinic. I have no EP training, do not place intracardiac devices and do not usually supervise this PA. Other more qualified individuals were also available and more qualified to staff the non-emergency clinical situation. I have an EEOc pending in Federal Court regarding the two Reprimands and temporary suspension

Action Taken: Temporary Suspension: I was temporarilty suspended because of my inability to place an order for an oral narcotic on an outpatient in CPRS. A review of cardiac catheterization procedures and Cardiology Consults found no issues and my privileges were reinstated. I have an EEOc pending in Federal Court with respect to the temporary suspension and reprimands
