Action Taken: FINAL DECISION AND ORDER FOR REMEDIAL EDUCATION. The Board reported this action to the Federation of State Medical Boards (FSMB) on July 5, 2016. For the avoidance of doubt, my Wisconsin license was not restricted or otherwise affected in any way. This is a NON-DISCIPLINARY action. This matter is CLOSED.

Action Taken: On May 5, 2017, the Hawaii Regulated Industries Complaints Office (RICO) opened an INVESTIGATION at the request of the Hawaii Medical Board in response to the Wisconsin Medical Board's non-disciplinary Final Decision and Order for Remedial Education, issued on June 15, 2016, and the Kentucky Board of Medical Licensure's Final Order, issued on March 20, 2017.

Action Taken: On September 28, 2017, I received the final executed CONSENT ORDER, which was approved on September 18, 2017. Pursuant to this order, my license to practice medicine in Illinois was FINED $500.00. This reciprocal DISCIPLINARY ACTION by IDFPR is based on the actions taken by the Wisconsin, Kentucky, and California Boards. This matter is CLOSED.

Action Taken: On 7/8/16, the Kentucky Board of Medical Licensure opened an INVESTIGATION in response to the WI Final Decision and Order for Remedial Education issued on 6/5/16, (underlying matter is Eversole case) and requested additional information. The Kentucky Board of Medical Licensure then concluded its investigation on 10/20/16, and a COMPLAINT was filed on 12/16/16. After a final hearing, on 3/20/17, the Kentucky Board of Medical Licensure issued DISCIPLINE in the form of a FINAL ORDER. My license was not restricted or otherwise affected in any way. This matter is CLOSED.
