Entity Taking Action: Kentucky board of licensure
Action Taken: The Virginia Board of Medicine Reprimanded me for signing a prescription for colleague of mine working together who needed refill on his medication and asked him to put same refill since I was busy with patient care at the time. The case was investigated while I was practicing in Virginia but the ruling came after I have transferred to Kentucky in 2013. The KENTUCKY BOARD BY POLICY AFTER BEING INFORMED BY ME, INVESTIGATED THE INCIDENT AND DECIDED TO GIVE ME THE SAME REPRIMAND THAT VIRGINIA BOARD GAVE PLUS FINE BECAUSE I DID NOT REPORT IT IN 10 DAYS AFTER RECEIVING THE INFORMATION FROM VIRGIN

Action Taken: 1. REPRIMAND based on my discipline in 2013 by the Virginia Board of Medicine 2. To provide a copy of the verification that I submitted to Virginia Board of Medicine and copies of any and certificates of completion of continuing medical education 3. Pay a civil penalty of $1,000

Entity Taking Action: Michigan Board of Medicine
Action Taken: I have not practiced Medicine in Michigan, I had active license there until January 2015 when I did not renew it. Recently communication from Virginia Medical Board relating to a disciplinary action from the Michigan Board and I contacted the Michigan Medical board for licensure and an employee of the office informed me that there was a letter sent to my old address and was returned. Since they did not get my response from that letter, the Board reached a decision to reprimand since I was reprimanded by Vriginia Board in 2013. Michigan Board reprimanded me.

Action Taken: I joined the program as a Second year resident and for the first 3 months I had difficulty adjusting to the system in comparison to other residents that have been in program for a year. Put on probation for poor first quarter evaluation. The probation was reversed 3 months later after an impressive performance.
