Entity Taking Action: Illinois Department of Financial and Professional Regulation
Action Taken: In 2016, I submitted my Application to the Illinois Department after receiving an employment offer for the position as an Emergency Room Director at a hospital in Illinois. Thereafter, the Illinois Department issued a notice of its intent to deny my Application. The Illinois Department’s basis for the notice was that I was suspended for one month during my residency program. I have always been honest and forthcoming about this issue, and the circumstances surrounding it. I currently hold active, unencumbered and undisciplined Physician’s Licenses in Virginia, Ohio, and Florida.

Entity Taking Action: Florida Medical Board
Action Taken: In October 2022, in response to the reprimand given by the State Medical Board of Ohio, the Florida Medical Board investigated the Ohio reprimand. The Florida Board issued a fine of $1,5000 and required I enroll in two CME courses. The first course is called 'laws, rules, and ethics course,' and the second is called 'medical records keeping.' I have enrolled in both of these courses. As stated previously I no longer work with the nurse with the IV hydration business and no new penalties were issued by Ohio or Florida.

Entity Taking Action: Ohio Medical Board
Action Taken: I entered into a consent agreement with the State Medical Board of Ohio on January 12, 2022. Under the terms, I was issued a reprimand, required to pay a fine of $1,000 and complete a medical records course. In 2019, I agree to serve as a supervising physician with a nurse acquaintance who started an IV hydration business. We administrated IV fluids, toradol, zofran, and reglan. I personally spoke to every patient but did not perform an in person or remote exam. The business did not hold a TDDD license. I no longer work with the nurse and have been licensed to practice in OH since 2014.

Action Taken: In April 2023, in response to the reprimand given to me by the Ohio and Florida Medical Board my Virginia Medical license was placed on probation until I complete a medical ethics course and fulfill my duties to the Florida Medical Board
