In 2005 when I applied for a WV license they did place a restriction on my license pending the successful completion of my contract with the PA PHP and this restriction was lifted in April 2007. This came about as a result of an iatrogenic physical drug dependency after surgery requiring inpatient treatment in 2002. I was monitored for 5 years by the PA PHP ending in April 2007 and that resulted in the lifting of that restriction. At no time did I do anything illegal nor unethical, nor did I ever practice impaired. There were no prior issues and none since.
