CA Medical Board granted Dr. Tavarez’s application for a license and reprimanded Dr. Tavarez based on a 2006 traffic infraction in New York that was not disclosed on Dr. Tavarez’s application for licensure. The Board found that Dr. Tavarez did not intentionally attempt to mislead the Board and issued a reprimand based on a California law that permits the Board to issue a reprimand to an applicant who has committed a minor violation. The reprimand does not restrict Dr. Tavarez’s license or ability to practice and is the least severe form of discipline available under California Board rules.
