Action Taken:
Issues with documentation on Thyroid surgery performed 2013-14. Consent order required a course on documentation. It required supervision of surgery and documentation for a year, as well as my license being on probation for a year. All of those requirements have been fulfilled. I also will refrain from performing any thyroid surgery.
