01/28/02: Acknowledged Acceptance of Voluntary Surrender of Medical License: Due to transformation of episodic migraines to chronic daily headaches in 1999, I sought the assistance of the NCPHP due to concern for OUD related to abortive prescription medications. I participated anonymously with NCPHP until an episode of recurrent use upon which I voluntarily surrendered my license on 1/24/02

Consent Order 09/13/02 - 11/22/05. I attended a 30-day chemical dependency program which also referred me to effective chronic pain management. As all issues had resolved, this was accompanied by a temporary medical license which was repeatedly extended at follow ups as I demonstrated successful completion of all requirements.
