Entity Taking Action: Colorado Medical Board
Action Taken: On September 9, 2021, the Medical Board approved a Stipulation and Final Agency Order in which I agreed to five years of probation. I agreed to the Order rather than proceed through a formal disciplinary hearing.

Entity Taking Action: Florida Medical Board
Action Taken: On August 26, 2022, the Florida Board of Osteopathic Medicine filed a Final Order which included issuing a letter of concern, a fine of 1,000.00 and a suspension of my Florida medical license. I am required to obtain an evaluation by the Professional Resource Network (PRN) which will be reviewed by the Board. Following review, the Board will determine my eligibility for reinstatement

Entity Taking Action: Illinois Medical Board
Action Taken: On October 7, 2022, I entered into a Consent Order with the Medical Board. My IL license will be placed on probation and I can petition to restore it once my probationary period with Colorado has been successfully completed. During this probationary period, I must submit quarterly reports to the IL Board and maintain compliance with all of the terms set forth by the Colorado Medical Board

Entity Taking Action: Kentucky Medical Board
Action Taken: On April 21, 2022, I entered into an Agreed Order with the Medical Board. This order mirrored the CO action of a five-year probation in addition to a $1,000 fine.

Action Taken: On February 22, 2022, I entered into an Agreed Order with the Medical Board in which I agreed to enter into a contract with the New Mexico Health Professional Wellness Program.

Action Taken: On August 3, 2022, the Osteopathic Medical Board ratified a consent order in which I agreed to a probation on my license for the duration of the probationary period set forth in the Colorado Order. During this period, I must maintain 100% compliance with the Colorado Provider Health Program contract I entered into on September 9, 2021

Action Taken: On March 16, 2022, I entered into an Agreed Order with the Medical Board which states that my Wisconsin Medical License was suspended. The suspension has been stayed which will continue as long as I comply with the CO Order.

Entity Taking Action: Blue Cross Blue Shield of Texas
Action Taken: On December 12, 2022, Blue Cross Blue Shield of Texas reported to the National Practitioner Data Bank that my contract with them was terminated effective September 23, 2022. This decision was based on the agreed order I signed with the Colorado Board of Medicine in September 2021. There has been nothing new to precipitate this action. Unfortunately, this report was the first time I had any knowledge that my contract with BCBS of Texas was in jeopardy. Had I been notified prior, not only would I have disclosed it as required, but I would also have submitted a request for an appeal.
